NCN Health has been offering the Care Finder Program for just over two years.

During this time, we have assisted many vulnerable older community members to access services and supports to keep them living independently and safely in their own homes.
Care Finder project officer Marie Doyle is eager to ensure Moira Shire residents know about the program and are comfortable contacting NCN Health for support.
“We are here to assist people who may not have family, friends or a strong support network to help them in accessing Aged Care services,” Ms Doyle said.
“The concern is that they may not know about this program and how we can help them.
“Care Finder’s aim it to help older vulnerable community members to access services and supports such as My Aged Care, Carer Gateway, Community Transport or health care and mental health services and supports as needed.
“With adequate services and supports in place, this enables people to continue living safely and independently in their own homes.”
The NCN Health Care Finder Program is based at Nathalia, Cobram and Numurkah but can service clients right across the Moira Shire.
When you first make contact with the Care Finder project officer, we will chat with you to determine your needs and, if necessary, organise to meet in person at your home or at an NCN Health Campus, depending on what suits you.
We will conduct an assessment with you to decide what services and supports you may need and start the process of assisting you to source these.
Ms Doyle said everyone’s needs are different.
“Some clients need only a small amount of guidance, for example some advice on what services and supports are available.
“While others need more intensive support, from assessment right through to when the services/supports are put in place.
The Care Finder Program will assist people who may not have family, friends or a strong support network to help them in accessing Aged Care services.

“Many of our clients find it hard to use a phone or computer, and I will remain in contact with them until they feel confident and well-supported.”

Clients are often referred to Care Finder by a health professional, but anyone can call to make an inquiry or appointment.
If you know someone in the community who you think could benefit from this service, you can call on their behalf, provided you have their consent.
Ms Doyle said while the Care Finder Program was aimed at the most vulnerable older people who have little family, friends or support network, she encouraged those who are struggling to understand how to access Aged Care services and supports to get in touch.
“Not everyone is eligible for the Care Finder Program, but we are happy to talk to you and assess your needs and go from there,” she said.
“The most important thing is to get the word out about the service, so the community know we are here and can help.”
